Appointments

We very happy that you're making a decision to invite Dr. Dobrovic to be your pain management physician and look forward to meeting you! Not only do we want to help resolve your pain, but we also want to make the process of working with us as easy as possible.

Getting Started

• Call 703.444.2592 to make your appointment
• You will be asked for a deposit charged to your credit card.
• Please download, print, and fill out this patient registration form.
• Bring the completed form with you to your first appointment.
• Your first appointment will be a minimum of an hour, but will be billed for one hour.
• You will leave your initial appointment with a prescription for a urine screening test.
• We will make an appointment to meet again in one week — after VPS receives the results of the urine screening test.
• Please note that no prescriptions for medications are issued on your first appointment.

Payment

• As noted on our Insurance page, VPS does not accept health insurance as payment.
• All visits require a deposit, payable by credit card (AmEx, Discover, MasterCard, or Visa).
• Initial consultations are billed for one hour.
  • Subsequent office visits will be billed in 20 minute increments.
  • Diagnostic tests and interventional therapies are additional costs and are discussed in advance.
• Payment of any balance is due when service is provided.
  • Payable with cash or credit card (AmEx, Discover, MasterCard, or Visa).
  • We do not accept checks.
• Cancellation with less than 24 hours notice or failure to show up for your appointment will result in the loss of your deposit.
